In our recent survey, campus customers provided suggestions for improving the Service Request Form. So, we've made some changes that will be effective August 4, 2017.

What's new:

  • a more intuitive user interface
  • easy identification of the correct form to use
  • ability to save form information for future use
  • confirmation that your Service Request Form has been successfully submitted
  • search by Service Request number, MSU NetID, date, or account number
  •  VPN access for off‐campus employees no longer required

Technical Requirements

Service Request Forms are required by Logistics for campus pickup of parcels and mail. We hope you find the updated form easier and more convenient to use.
